Forest MD Rewards – Terms & Conditions
- General Terms
1.1. IYAC Rewards is a customer loyalty and reward programme (“Membership Programme”) by IYAC Holdings Pte Ltd and Forest MD Inc Pte Ltd (collectively “Company”), and is available to individuals who meet the requirements and have successfully enrolled in the Membership Programme (“Members”). The Company reserves the right to change the name of the Membership Programme &/or the technology used to host the Membership Programme (“Rewards Platform”) at any time, with or without notice.
1.2. By participating in the Membership Programme, Members agree to accept and be bound by the Terms and Conditions (“T&C”) of the Membership Programme, which T&C for the avoidance of doubt may be stated herein or in any other communication of the Membership Programme. The Company reserves the right to amend or update these Terms and Conditions (T&C) at any time, with or without notice.
1.3. The Membership Programme is valid in Singapore at the physical store and online stores www.iyacmedspa.com and www.forest-md.com of the Brand. The Membership Programme is not valid for transactions made on third-party delivery/marketplace platforms, or at other locations/platforms designated by the Company at its sole discretion.
- Eligibility & Enrolment
2.1. To participate in the Membership Programme, individuals must sign up as a Member for the Membership Programme and maintain a user account on the designated Rewards Platform &/or other designated platforms.
2.2. The Member warrants that all information and personal data submitted in application for the Membership Programme is accurate and true; and agrees to the collection and use of personal data in accordance with the Company’s Data Privacy Policy.
2.3. On the date of successful enrolment (“Join Date”), Members will receive confirmation of the same and access to the benefits &/or obligations of the Membership Programme (“Membership”).
- Membership Benefits
3.1. The benefits of the Membership Programme (“Benefits”) refer to the ability to belong to a membership tier; to earn points; to redeem rewards using points; to receive perks (e.g. gifts, treats, savings, vouchers, privileges); and other benefits not stated herein but to be determined solely by the Company.
3.2. All Benefits are subject to their own T&C, and cannot be used together with other vouchers, rewards, promotions, offers or discounts, unless otherwise stated.
3.3. Benefits that are offered with an expiry date shall be utilised before the expiry date, failing which they will be forfeited; there will be strictly no extension of the expiry date, nor compensation for the forfeiture, unless otherwise stated.
3.4 All Benefits are strictly non-replaceable and non-transferrable, unless otherwise stated; the Member is solely responsible for his/her membership and the utilisation of his/her Benefits, and the Company shall not be responsible for any unauthorised use of the same.
3.5. All Benefits cannot be exchanged for cash, whether in part or in whole.
3.6. The Company reserves the right to deny the utilisation of any or all Benefit(s) by any Member for suspected misuse, abuse or fraud, among other reasons, of the Membership Programme at its sole discretion.
3.7. The Company reserves the right to amend, update &/or withdraw any or all Benefit(s), or part thereof, at any time at its sole discretion, with or without notice.
- Membership Tiers
4.1. The Membership Programme consists of four tiers of Membership (Silver, Gold, Platinum and Jewel) that each carry their own range of Benefits. To upgrade into / remain at any given tier, Members must achieve a Minimum Annual Spending Requirement (“MASR”).
4.2. The MASR defines the minimum cumulative spending that is achieved over a period of 12 months (“12-month Period”). In the first year of membership, the 12-month Period starts from the Join Date.
4.3. On the last day of the 12-month Period (“Tier Anniversary Date”), if a Member achieves the MASR for his/her existing tier, the Member will automatically be kept at his/her existing tier; if a Member does not achieve the MASR for his/her existing tier, the Member will automatically be moved into a lower tier that corresponds with the level of his/her spending. For subsequent years of membership, the 12-month Period restarts on each Tier Anniversary Date.
4.4. Within a 12-month Period counted from the Join Date or Tier Anniversary Date (whichever is applicable), on the date that a Member achieves the MASR for a higher tier (i.e. Gold), the Member will automatically be moved into that higher tier (“Upgrade-to-Gold Date”). At the end of the 12-month Period counted from the Join Date or Tier Anniversary Date, if the Member has not achieved the MASR for an even higher tier (i.e. Platinum), then the Upgrade-to-Gold Date will automatically become the Tier Anniversary Date, from which subsequent 12-month Periods will be counted.
4.5. The Company reserves the right to amend the MASR, the 12-month Period, the Tier Anniversary Date, and the Benefits of each tier at any time at its sole discretion, with or without notice.
- Earning Points
5.1. Accumulation
5.1.1. Members may earn points on Net Eligible Spending made at the physical store and online stores (www.iyacmedspa.com and www.forest-md.com).
5.1.2. For eligible transactions that are discounted by or partially paid with Physical Vouchers, Online Gift Cards, or any other value-bearing token as designated by the Company at its sole discretion, Points may be earned only on the net payable amount (rounded down to the nearest whole Singapore dollar) after deducting the discount and/or partial payment (“Net Eligible Spending”).
5.1.3. Members may also earn points by performing actions (e.g., Refer a Friend, Leave a Review) that have been assigned points within the Membership Programme.
5.1.4. During the Member’s birthday month, the first purchase made (either online or in-store) will earn 2x points—that is, twice the number of points that would otherwise have been earned on a regular transaction.
5.1.5. In the event of any order changes, including cancellations, refunds, exchanges, or amendments made after points have been issued, the corresponding points will be reversed or adjusted. Such adjustments may also result in movement of the Member’s reward balance and/or membership tier status.
5.2. Validity
5.2.1. Points are valid for 18 months from the end of the month in which they were earned. For example, 100 Points earned on 15 Mar 2025 and 88 Points earned on 22 Apr 2025 will expire on 30 Sep 2026 and 31 Oct 2026, respectively, if unused.
5.2.2. Upon expiry, points will be automatically forfeited. There will be no extension of validity or reinstatement of expired points, unless otherwise stated by the Company.
- Redeeming Rewards
6.1. Redemption
6.1.1. Rewards and perks of the Membership Programme include vouchers and gift privileges. Rewards are redeemed by the Member using points that she has earned. Perks are given by the Company without requiring points, typically when a Member reaches a milestone (e.g., birthday, tier upgrade).
6.1.2. Members may use the rewards and perks at the physical store and online stores (www.iyacmedspa.com and www.forest-md.com).
6.1.3. A maximum of up to two (2) reward vouchers may be applied in a single transaction/redemption. Rewards cannot be combined with other offers or promotions and are non-transferable, non-replaceable, and non-exchangeable for cash or other products, unless otherwise stated.
6.2. Validity
6.2.1. Rewards and perks are valid for the Period stated on their respective T&C. Upon expiry, the rewards shall be forfeited; there will be strictly no extension of the expiry date, nor compensation for the forfeiture, unless otherwise stated.
- Cancellation of Membership
7.1. Members may opt out of the Membership Programme at any time by contacting the Company at rewards@iyacmedspa.com
7.2. The Company reserves the right to close Membership accounts that have not incurred any spending for over 3 years.
7.3. The Company reserves the right to remove any Member from the Membership Programme at its sole discretion, without any liability to the removed Member.
7.4. Upon cancellation of membership for any reason, the Member’s existing membership tier, unused Points, and unused rewards will be forfeited.
7.5. In the future event that the membership is successfully re-opened again, the Member’s previous membership tier, unused Points, and unused rewards will not be reinstated.
- Liability & Dispute Resolution
8.1. The Company is not liable for any loss or damage arising from participation in the Membership Programme and utilisation of the Benefits, including the earning of points and the redemption and use of rewards.
8.2. Any disputes relating to the Membership Programme will be resolved in good faith directly between the Member and the Company; through non-judicial mediation; and failing which, in accordance with the laws of Singapore and the exclusive jurisdiction of the courts of Singapore.
